WILL I NEED TO BRING FOR MY WIC
APPOINTMENT?
You will need to bring the following
items to every scheduled appointment:

Medicaid card or proof of income
(recent check stub, recent child
support stub or unemployment stub,
most recent W2s, etc.)

Proof of identity for the applicant
(driver
license, birth certificate)

Proof of address (driver license or
a bill or
rent receipt)
-
Proof of pregnancy, if available
(for
pregnant women)
-
Immunization record (for children)
-
The person being certified
-
Legal
documents if a foster parent is
applying for a child
(Note:
Appointments can take up to 2 hours)
